Scheduling Follow-Up Phone Consultations for Prescription Refills
After completing your "in person" exam, ideally, follow-up exams will be conducted over the phone every 60 days with a brief six month follow-up visit in person.
We will remind you, via e-mail, approximately two weeks prior to the end of the 60 day period about scheduling your follow-up phone consultation.
This e-mail reminder serves two purposes:
- It will contain a Patient Assessment form that you fill out telling us how you're doing, and
- It will help keep you from running out of medications by reminding you that refills, if necessary, are almost due.
After receiving our e-mail alert you simply fill it out, send it back to us via email at patientsupport@ppmmd.com, or fax to: 1-209-754-0231
Upon completing the form, call us at 1-209-754-0248 and make your appointment.
On the day of your telephone consult you will receive a call from your PPMMD provider and, at that time, your existing prescription will, if necessary, be refilled for another 60 days.
