New Patient online scheduling for Doctor Consultations

Patients must first complete PPMMD’s secure online registration process. After you complete the registration process, send PPMMD up to 20 pages of your current records, if available. You will then be contacted to schedule an "in person" consultation. In-person consultations are required once or twice a year depending on circumstances and must occur prior to receiving any prescriptions. (Please see patient agreement (pdf)).

Scheduling Follow-Up Consultation

Once patients have satisfied their in-person consultation requirement, they can schedule a follow-up in-person or telephone consultations depending on the treatment plan recommended by their PPMMD provider. Remember, you do not have to wait until your refills run out to schedule your follow-up consultations. Always schedule early to avoid delays of your prescription medication.
