Patients must first complete PPMMD's secure online Patient Registration. This appointment is for a patient who is going to have their first face-to-face in-person visit with a PPMMD physician.
After you have completed the Patient Registration and prior to scheduling your appointment fax or mail the following:
Once patients have satisfied their in-person consultation requirement, they can schedule a follow-up in-person or telephone consultation depending on the treatment plan recommended by their PPMMD provider.
